Physiological Role
Neutrophils are born in the bone marrow and circulate for only a few hours before reaching the tissues. They form the first line of the innate response, the one that mobilises within hours without prior learning. Their number in blood therefore reflects a permanent balance between production, exit into the tissues and destruction.
Their main function is phagocytosis, the ingestion and then destruction of micro-organisms. They also release enzymes and reactive oxygen species, and deploy nets of DNA that trap foreign agents. Those same tools, activated over a long period, sustain low-grade inflammation.
A large share of neutrophils stays attached to the vessel wall, as an immediate reserve. Physical effort, acute stress or a dose of corticosteroids detach that reserve within minutes and raise the circulating number. This mobilisation reflects no new production, which explains the marker's volatility from one draw to the next.

Biological Significance
The neutrophil number reads first as a level of solicitation. High values accompany ongoing inflammation, acute or background, and are met in smokers as well as during pregnancy. Low values follow reduced production, increased consumption, or simply a different distribution between the blood and the vessel wall.
Part of the variation is constitutional and carries no bearing on health. The Duffy-null phenotype, predominant among people of African ancestry, comes with a lower circulating number at identical health status and with no lesser defence against infection. Recent work established a dedicated reference interval for this phenotype, markedly shifted downward.
The most informative reading is longitudinal. A value stable from one panel to the next says little; a steady drift over several years accompanies the immune remodelling described by longevity cohorts. It is this slope, rather than an isolated figure, that follow-up seeks to make visible.
Neutrophils are read with lymphocytes rather than alone. Their ratio, followed separately on this site, sums up in one number the balance between the two sides of the immune response. hs-CRP brings a third angle, independent of the white cell line.
Influencing Factors
Infectious episode. A recent bacterial infection raises the neutrophil number, often sharply, and the elevation persists for several days after the signs disappear. A draw taken in that window does not reflect the background state.
Physical effort. Intense exercise mobilises within minutes the reserve attached to the vessel wall. The elevation is immediate, transient, and fades within hours.
Acute stress and corticosteroids. Physical or psychological stress, like a dose of corticosteroids, detaches that same reserve. The effect assumes no inflammation.
Tobacco. Active smoking raises the neutrophil number durably, in proportion to consumption. It is one of the few modifiable factors whose effect reads directly on this marker.
Ancestry. The Duffy-null phenotype comes with a lower number without consequence for health. This particularity concerns a large share of people of African ancestry, and a specific reference interval has been published to account for it.
Pregnancy. The neutrophil number rises progressively through pregnancy, then returns to its earlier value after delivery.
Age. The circulating number increases slowly across the decades. Ageing cohorts describe this drift as a component of immune remodelling.
